Patient Information

We strive to keep our office charges affordable and will work with you to keep costs down for any procedure, without sacrificing good care. Our fees are comparable to other specialists in the area.

For your convenience, we will bill your insurance for you. Any initial fee (or co-payment) required by your insurance, and the fee for any service not covered by insurance, is payable at the time of service.

Because we participate in most insurance plans, we need to know information about your plan to serve you best.

We need:

• A copy of your valid insurance card
• Insurance billing address and phone number
• Please inform office staff of any changes of your health insurance coverage

Payments for hospital and laboratory services should be made directly to them, and not to our office.

Please feel free to discuss our fees and services with us at any time. If a misunderstanding about charges occurs, please call us. If a financial hardship develops, our business staff is happy to help you work out special payment arrangements.

Your Appointment

We see patients by appointment only, and we always strive to serve you on time. Emergencies or deliveries, however, will occasionally disrupt our schedule. When this happens, we will let you know how long we expect your wait to be, or we are happy to reschedule your appointment for your earliest convenience. If you are unable to keep your appointment, we request you give us 24 hours notice. One of our other patients, who can be moved into the open time will appreciate your thoughtfulness.

We have provided a way for you to complete your new patient forms prior to arriving for your appointment. Simply print the forms, complete them and bring them to your appointment for presentation at our front desk.
